- Are there any fee's involved with membership?
No, membership is free and completly voluntary.
- How can I set up an Intake?
Just contact us to make an appointment with Megan, our Membership Unit coordinator.
- How do I get to Horizon House using public transportation?
Horizon House is on the 136/137 bus line which begins in Malden Center and the Purple line. We are right outside Wakefield Sq. on Water St across from Omelet Headquarters and DMH.
- How much is lunch?
Lunch is served at Horizon House daily at 12 and the cost is $2.00 for members.
- How do I become a member?
Please check out our very informative "How to become a member" page.
- How do I make a referral?
If you would like to refer someone to Horizon House, please call us and we will fax a copy of our referral form, which you in turn, can fill out and fax or mail back to us. Our criteria for membership is having a severe and persistant mental illness, not being a current danger to self or others, and residing within the towns we serve. Any mental health professional can make a referral such as therapists, DMH Case Managers, PCP's and psychiatrist's.
